Stable patients: one Medical Report or Progress Note a month is enough. Write another whenever the condition or the plan changes. Saved entries cannot be edited.

Fix data after the 28 Jul schedule update — moves each patient's recorded data to their new machine slot instead of leaving it behind.
Week separation upgrade — gives every date its own record so a late entry can never overwrite a previous week. Run once, while nobody is entering data.
Use this at the start of a new week once the team has saved and reviewed the previous week's data. This does not happen automatically.

Every closed week is archived here and kept indefinitely. Supervisors can restore an individual record back into the live week.
